  Attenuated sensitivity to the emotions of others by insular lesion

  The insular cortex is involved in interoception and emotional awareness. We examined emotion recognition and interoceptive accuracy in three patients with right insular and adjacent lesions. Participants identified morphed facial expressions of anger, sadness, disgust, and happiness and performed a heartbeat perception task. Patients tended to perceive highly arousing emotions, such as anger, as less arousing emotions, and one showed markedly reduced interoceptive accuracy. These findings suggest that insular lesions reduce emotional sensitivity across emotion categories rather than causing a specific deficit for disgust, supporting a role of the insula in emotional saliency, interoception, and emotional awareness.
